Two of the most commonly used viscoelastic hemostatic assays, thromboelastometry (ROTEM) and thrombelastography (TEG), have proven to decrease allogenic blood transfusions with cost reduction and possibly decrease mortality and morbidity in cardiac, trauma, and other bleeding patients. This article compares the two devices to provide guidance on the selection and use of these monitoring systems.

The two clinical studies were in liver transplantation and in cardiac surgery, another was an experiment using commercially available plasma and the last was a head-to-head comparison of the technical aspects, ease of use and costs [ 7 , 10 – 12 ]. This head-to-head comparison of TEG ® and ROTEM ® in identification of hyperfibrinolysis in 37 adult LT patients demonstrated that ROTEM ® assays were more sensitive than k-TEG ®. Furthermore, FIBTEM ® was more sensitive than EXTEM ® in identifying hyperfibrinolysis. Strong to very strong correlations were observed between corresponding TEG 6s and ROTEM Sigma parameters. However, absolute values showed significant differences for most of the measurements. Furthermore, FIBTEM ® was more sensitive than EXTEM ® in identifying hyperfibrinolysis.

It is a modification of traditional thromboelastography (TEG). Le TEG comme le ROTEM font parties des méthodes qui, loin de révolutionner la médecine, permettent de la rendre plus efficiente et peut être plus intéressante aussi pour nous. TEG 6s functional fibrinogen level was compared with plasma fibrinogen concentration, measured using the Clauss method. Correlations were classified as weak (Spearman correlation coefficient 0.20 to 0.39), moderate (0.40 to 0.59), strong (0.60 to 0.79) or very strong (≥0.80).

While TEG operates by spinning the cup itself, ROTEM devices spin the pin while holding the cup stable. 4 The devices generate a graph of the measured elasticity of the blood, expressed in millimeters (mm In both methods, whole blood is inserted into a cup, which must be pre‐warmed to 37°C.

TEG parameters are influenced by the presence of heparin/heparinoid substances. In fact the main difference between TEG and ROTEM is the bit which rotates (TEG rotates the cup, and ROTEM rotates the pin).

However, they provide similar information on clot formation.

2020-09-24 2019-03-06 Thromboelastometry (TEM), previously named rotational thromboelastography (ROTEG) or rotational thromboelastometry (ROTEM), is an established viscoelastic method for hemostasis testing in whole blood. It is a modification of traditional thromboelastography (TEG). TEM investigates the interaction of coagulation factors, their inhibitors, anticoagulant drugs, blood cells, specifically platelets TEG and ROTEM: The Basics TEG and ROTEM are close cousins that assess in real time the entire coagulation process, including fibrin formation and clot rate, strength, stability, and lysis. These two viscoelastic measures of whole blood provide essentially the same information on clot formation, kinetics, and strength, though they operate slightly differently. •TEG/ROTEM in coagulopathy, transfusion and mortality in trauma •55 studies (12,489 patients) •TEG/ROTEM were SN and SP for early detection/prediction of: •Hypocoagulability •Transfusion needs •Mortality Comparison of Thromboelastometry (ROTEM 2015-02-16 Travis Morrison-McKell MD, B.
